On admission, following being welcomed, introduced and shown around as appropriate, the Care Planning process will be further developed, carrying on from the pre-admission assessment.
This process will indude input from:
- The Resident / Care Representative
- Medical Officer / GP
- Health Care Assistants
- Activity Co-ordinator
- Kitchen Manager
The Care Planning process comprises of assessing both the care and social needs of the resident, planning how these needs are going to be met, delivering the care and evaluating the care given. A review of the Care Plan will be prompted following any changes in the resident's personal or social needs/circumstances and will be reassessed and updated no less frequently than at three-monthly intervals.
Care plans are made available to residents/care representative in a format appropriate to the individual.
